We believe that leaving treatment without a clear plan is one of the greatest risks to sustained recovery.

That is why aftercare planning begins during your residential stay, not on the day you leave. Our clinical team collaborates with you to identify the resources, relationships, and routines that will support your continued growth. We take the time to understand your living situation, employment, family dynamics, and personal goals so your aftercare plan is genuinely tailored to your life.

You will never be discharged without a roadmap. Every client who leaves Chelsea Manor does so with a written aftercare plan, provider referrals, and the knowledge that our alumni team is available whenever they need support.

Personalized Aftercare Plan

A detailed, written plan developed collaboratively with your clinical team. It outlines your ongoing treatment goals, recommended services, coping strategies, trigger management, and daily structure recommendations specific to your life circumstances and recovery stage.

Outpatient Provider Referrals

We connect you with vetted outpatient therapists, psychiatrists, and counselors in your area who specialize in addiction and co-occurring disorders. Warm handoffs are coordinated so you have continuity of care from the moment you leave our facility.

Support Group Connections

Whether you prefer 12-step meetings, SMART Recovery, Refuge Recovery, or other peer support models, we help you identify groups near your home and establish a meeting schedule before discharge. Community connection is one of the strongest predictors of sustained recovery.

Alumni Resources

Chelsea Manor alumni have access to ongoing support including periodic check-ins, alumni events, and a community of graduates who share the bond of recovery. Our alumni program helps you stay connected and accountable as you navigate life after treatment.

Relapse Prevention Strategy

Your aftercare plan includes a detailed relapse prevention strategy developed during your residential stay. This covers personal trigger identification, early warning signs, coping techniques, emergency contacts, and a step-by-step action plan for high-risk situations.

Continuing Care

The transition from residential treatment to daily life is one of the most critical periods in early recovery. We take this transition seriously. Aftercare planning is woven into your treatment from the first week, not treated as an afterthought on the day of discharge.

In the weeks before discharge, your clinical team reviews your progress, updates your treatment goals, and finalizes referrals. We coordinate with your outpatient providers so they have the context they need to continue your care effectively. If sober living is appropriate, we help identify reputable options and facilitate the placement process.

Family involvement is also a key element of the transition. When appropriate, we facilitate family sessions to educate loved ones about the recovery process, set healthy boundaries, and establish a supportive home environment. A strong family system can be one of the most protective factors in sustained recovery.

After you leave, our door remains open. Alumni are welcome to reach out to our clinical team, attend alumni events, and access ongoing support resources. Recovery is a long-term commitment, and our relationship with you does not end at discharge.
